“Where Do I Pick Up My PWD Application Forms?”
Here's the answer to our most frequently asked question at the VDRC.
The short answer (for more detailed information, see link below, “More detailed answer”):
In order to get the PWD application booklet you must register online or by phone:
- To register by phone, call 1-866-866-0800 (select option 2, then option 1, and then option 2).
- To register online, create a My Self Serve account. You will need a current email address and your Social Insurance Number (SIN).
